Submit a captured thread for review POST

Accepts a capture from the browser extension and returns the pending review item, building a proposed project mapping from it. Deduplicated on (team, source, external_id): a repeat capture of the same thread returns the existing item with 200 rather than creating another, so the extension can retry safely.

Upload a file POST

multipart/form-data with one `file` part and an optional `category`. The bytes are encrypted at rest. Honors `Idempotency-Key`: a retry with the same key and the same file replays the original response; the same key with a different file returns 422. Max 25 MB.
